An emergency travel document is only issued to Filipino citizens in and emergency to allow one way travel to the Philippines ONLY. This cannot be used to travel to any other country. The applicant cannot return to Ireland on this emergency travel document and will need to apply for a new Philippine passport in the Philippines. The document is only valid for 30 days and will only be issued two weeks in advance of definite travel plans.

Note: Failure to email the documents in advance will result in your appointment being cancelled and allocated to another person, you will then have to rebook a new appointment.

ADULT APPLICANT – Copy of your expired Philippine passport. – In the case of lost passport provide a clear image of the data page of the lost passport. If the image is not available you must present an ORIGINAL PSA birth certificate along with two valid forms of identification. – A complete new passport application form available from the Philippine Embassy London website. – An affidavit explaining why your require a travel document. – Definite travel plans within two weeks of appointment.

– Three passport photos 

 

MINOR / FIRST TIME APPLICANT – Copy of your expired Philippine passport. – First time applicants an original PSA birth certificate or a Report of Birth notoried by the Philippine Embassy dated less that one year before intended travel. – In the case of lost passport provide a clear image of the data page of the lost passport. If the image is not available you must present an ORIGINAL PSA birth certificate along with two valid forms of identification. – A complete MINOR passport application form available from the Philippine Embassy London website. – An affidavit explaining why your require a travel document signed by the parent or guardian. – Definite travel plans within two weeks of appointment.

– Three passport photos.
– Valid passport of the parent or guardian and a photocopy of the data page
